Father’s Day looks a little different when your kids aren’t little anymore. Gone are the days of fingerprint tie gifts and macaroni-noodle cards (though I still have every one of those tucked away). Now, with four older kids—18, 17, 14, and 12 (be still, my heart… time truly does fly)—I’ve had to get a little more creative. So we created a Father’s Day printable for teens and older kids to give.

For years, we’ve used this Father’s Day Questionnaire. It’s always my favorite tradition— to give the same one year after year. It’s always been the perfect gift: adorable, fun, simple, and hands-down one of the best gifts they’ve given, year after year, since they were little.

But— it’s time for the next chapter & tradition. Now that our kids are older, they are ready for something a little more in-depth. Instead of questions like, “How old is dad?” and “How tall is dad?”, they are ready to move on to questions like, “The best thing that you taught me was…”

So, I created a new All About My Dad Questionnaire — Teen version. This new Father’s Day Survey from Teens (with input from OUR teens about what questions to include) ended up being the perfect gift!

All About Dad Printable: A Teen’s Gift to Dad

This printable Father’s Day questionnaire was simple, sweet, and just the right mix of heartfelt and fun. The best part? Our teens actually wanted to fill it out.

I also created Father’s Day Surveys for their grandfathers, and for “You,” which encompasses anyone they hold dear. All of the printables are included in one Father’s Day Printable Bundle (downloadable packet):

  • All about Dad (from Teens)
  • All About Grandfather (from Teens)
  • All About YOU (from Teens. This one works for anyone who has played a significant role in your child’s life.)

Father’s Day Questionnaire from Teen or Older Child

I originally created these printables because I couldn’t find anything that really “fit” the stage our family is in now, with teens and young adult children. Most Father’s Day printables are perfect for preschoolers or elementary-aged kids, but lack a little depth when it comes to an teen or young adult filling them out. After searching for an ‘All About My Dad’ questionnaire teens could fill out, I came up empty-handed every time. It was impossible for me to find even a basic Father’s Day printable from teens, so I knew I needed to create my own. 🙂

Instead of a cute Father’s Day craft, we sat down with these “All About My Dad” questionnaires and let the kids take the lead. Each one filled it out on their own—and then we gifted them to the three men who play a big role in their lives: their dad, their grandfather (my dad), and their uncle.

Here are just a few of the questions that your teenagers or older children can fill out for dad or grandpap:

  • “If I could give you a whole day of doing what you love, I’d give you a day of ______.”
  • “Your dream car is a ____.”
  • “I’m grateful you taught me_______.”

Mickey smiled, laughed, and agreed with everyone’s answers (they know him well!)

It’s Not Complicated. It’s Just Real.

Teens may not always say how much they appreciate their parents, but they do. They are watching. They are listening. They are paying attention. It’s the reason that I wrote about the things no one tells you about raising a teenage boy
Teenagers might not come right out and say that they care, but they do. They might not say that they notice, but they do.

This printable gives them the chance to say it.

While I’m sure they won’t remember all of these things when they’re older, they’ll remember how the things you’ve said and done for them made them feel loved, valued, and safe. This printable highlights some of those moments so they can be remembered.

Father’s Day printable from teen

This Father’s Day printable from a teen shares their favorite memories.

As a mom, what I want most is for my kids to stay connected to the people who love them. This small tradition helped them slow down for a moment and reflect on just how much those relationships mean. A simple Father’s Day printable from your teen that feels genuine and personal is perfect for focusing on the reason for Father’s Day— to share our memories & appreciation for dad, grandpap, uncles, etc.

That’s the kind of gift that lasts far longer than socks or grilling tools. It’s a meaningful Father’s Day printable gift made by teens, for dads, and it doesn’t get much better than that. 🙂
